Create & Cultivate (“we,” “us,” or “our”) is a nonprofit organization recognized as tax-exempt under Section 501(c)(3) of the Internal Revenue Code, EIN 88-1557753, with its principal mailing address at 2413 Kittyhawk Dr, Plano, TX 75025-6093.

Many states require charitable organizations that solicit contributions to register with a state agency and to include specific disclosure language in their solicitations. Registration in a state does not imply that the state endorses or approves us.

02Action required before publishing

TODO(legal): this organization must determine every state in which it solicits contributions, register where required, and insert each state's exact statutory disclosure language together with its registration number. The entries below are structural placeholders, not legal disclosures, and several states mandate precise wording that must be reproduced verbatim from the statute.

• Florida — registration number and the state's required toll-free disclosure statement.
• Maryland — copies of documents and information filed with the Secretary of State are available on request.
• Michigan — charitable organization registration number.
• Mississippi — official registration and financial information statement.
• New Jersey — registration and financial documents filed with the Attorney General.
• New York — a copy of the latest annual report is available from the Attorney General's Charities Bureau.
• North Carolina — financial information and a license may be obtained from the State Solicitation Licensing Branch.
• Pennsylvania — registration information available from the Department of State.
• Virginia — a financial statement is available from the Office of Charitable and Regulatory Programs.
• Washington — financial disclosure information available from the Secretary of State.
• Wisconsin — a financial statement of the most recent fiscal year is available on request.

We are located in State of Texas. Texas does not require general charitable solicitation registration for most nonprofit organizations, though specific categories of solicitation are regulated.
